Congratulations on your decision to participate in an exercise program! Here at HPC, you will accomplish your training goals faster, safer, and with maximum benefits.
In order to maximize progress, it will be necessary for you to follow program guidelines during supervised and (if applicable) unsupervised training days. Remember, exercise and healthy eating are EQUALLY important! During your exercise program, every effort will be made to assure your safety.
However, as with any exercise program, there are risks, including increased heart stress and the chance of musculoskeletal injuries. In volunteering for this program, you agree to assume responsibility for these risks and waive any possibility for personal damage. You also agree that, to your knowledge, you have no limiting physical conditions or disability that would preclude an exercise program.
By signing below, you accept full responsibility for your own health and well-being AND you acknowledge an understanding that no responsibility is assumed by the leaders of the program.
It is recommended that all program participants work with their personal trainer two (2) times per week. However, due to scheduling conflicts and financial considerations, a combination of supervised and unsupervised workouts is possible.
I agree that Weston Zierer (The Human Performance Center) or any affiliates associated with the property of the gym location, shall not be liable or responsible for any injuries to me resulting from my participation in the fitness program (whether at home, outdoors, or at a corporate, commercial, residential or other fitness facility) and I expressly release and discharge Weston Zierer and HPC Gym affiliates, from all claims, actions, judgments and the like which I or my heirs, executors, administrators or assigns may have or claim to have as a result of any injury or other damage which may occur in connection with my participation in the fitness program, excepting only an injury caused by the gross negligence or intentional act of such person or persons. This Release shall be binding upon my heirs, executors, administrators and assigns.
I understand the results of any fitness program cannot be guaranteed and my progress depends on my effort and cooperation in and outside of the sessions.
I understand that all personal training rates are based on 55-90 minute sessions (excepting specialty programs) and should I arrive late, there is no guarantee I will receive the full session with my trainer. In return, if my personal trainer is late for a session, I will still receive the full session time.
I understand that Weston Zierer (The Human Performance Center) bills it's training clients on a pre-pay basis. Once my trainer and I have decided upon the number of sessions or specialty program I will purchase, payment must be made within the 1st week of training or before the sessions are conducted. Checks are to be made payable to Weston Zierer. I understand that all training sessions are non-transferable and non-refundable. I also understand that all training sessions must be redeemed within one year of purchase.
I understand that Weston Zierer (The Human Performance Center) operates on a scheduled appointment basis and thus, requires that I provide 48 hours notice when canceling an appointment. No charge will be levied should I cancel with MORE than 48 hours notice given. Should I cancel a session within 24-48 hours prior.
The Human Performance Center's training is based on accountability. When purchasing monthly training packages you are paying to reserve a designated training time slot and NOT a session(s). It is solely your responsibility to be present and on time if not early for your training session. HPC Gym will not fill your time with another client(s) if you do not show up to your training session. Missing training days without 24-48 hour notice or legitimate reason will result in a missed session. You will not get your missed session on the backend of your training session. If your trainer wants to negotiate a time to make up sessions that’s completely between the client and trainer and HPC is not responsible for missed sessions.
The client agrees that The Human Performance Center operates on a scheduled hourly appointment basis for private training packages. Therefore, when canceling an appointment, I am required to provide MORE than 24-hour notice, for which no additional levy shall be incurred.
I hereby grant permission to The Human Performance Center to use photographs and/or video of me taken during training sessions or classes and or testimonials for use in publications, news releases, online, and in other communications related to the mission of The Human Performance Center.
I understand that during a personal training session, my trainer may have to use touch training to correct alignment and/or to focus my concentration on a particular muscle area to be targeted. If I feel uncomfortable or experience any type of discomfort with touch training, I will Immediately request that my trainer discontinue using this technique.
